The Weekend Baylor Hospice RN is responsible for planning and delivering patient care using the nursing process, which includes assessment, planning, interventions, implementation, and evaluation, while interacting effectively with patients, families, and the interdisciplinary team. Essential duties involve managing patient care, coordinating all services, providing skilled nursing care in the home, documenting services, and ensuring continuity of care with an emphasis on pain and symptom management.

The weekend baylor hospice rn plans and delivers care to patients utilizing the nursing process of assessment, planning, interventions, implementation, and evaluation; and effectively interacts with patients, significant others, and other interdisciplinary team members while maintaining standards of professional nursing and clinical competency. Provides appropriate palliative and hospice care through assessment, teaching, counseling and direct care to client and/Or family members. Coordinates care with other community resources and other team members.
An active american health association bls certification;: Associate’s degree or bsn (Preferred) from an accredited school of nursing;
At least 1 year of direct care nursing experience, preferably the area of public health or home care nursing; oasis and/Or epic experience is preferred.

$83,200.00 USD - $129,626.00 USD

Candidates must possess an active, valid New Hampshire RN license and an active American Health Association BLS certification. A minimum of 1 year of direct care nursing experience, preferably in public health or home care nursing, is required, though an Associate's Degree or BSN is preferred.
