The role involves conducting telehealth follow-up visits for psychiatric medication management in geriatric patients, monitoring treatment response, and performing cognitive screenings and assessments. Responsibilities also include developing initial care coordination and behavioral support plans while collaborating closely with nursing staff.
Candidates must be licensed and certified as a Physician Assistant or PMHNP in Texas, with experience in geriatric psychiatry or dementia care being strongly preferred. Comfort with telehealth platforms and strong clinical judgment regarding medication safety in older adults are essential qualifications.
Licensed and certified as a Physician Assistant or PMHNP in Texas (required) Experience in geriatric psychiatry, long-term care, or dementia care strongly preferred Experience with cognitive assessment tools and behavioral management strategies Comfortable delivering care via telehealth platforms Strong clinical judgment related to polypharmacy, fall risk, and medication safety in older adults Ability to work collaboratively within a team-based care model
