The role involves developing, implementing, and revising occupational therapy plans of care with measurable outcomes in collaboration with clients, families, and physicians. Responsibilities also include implementing client care activities, assessing intervention effectiveness, and coordinating discharge plans with relevant parties.
Candidates must possess a Master's and/or Doctorate in Occupational Therapy from an accredited institution, along with a current O.T. License in the State of Maine and a current CPR Card. One year of clinical Occupational Therapy experience and Home Health experience are preferred, and a reliable, insured automobile is mandatory.
Incredibly flexible Health Insurance plans Education Reimbursement Paid time off Opportunities for internal promotions What You’ll Do: Develops occupational therapy plan of care with realistic/measurable outcomes in collaboration with the client/family and physician including discharge goals and according to Organization protocols. Provides education and promotes client’s rights and responsibilities for informed decision making in the plan of care. Develops, implements, and revises a plan of care that incorporates goals consistent with the client’s age, condition and needs. Implements client care plan activities based on treatment goals towards independent living for the client/family. Continually assesses the client’s needs for occupational interventions and evaluates the effectiveness of the interventions on the occupational visit report. Refers to internal and external community resources to meet client outcomes when appropriate. Initiates diagnostic, preventive and rehabilitative procedures as appropriate to the client’s care & safety to assist in the development and revision of Plan of Care. Evaluates and documents progress toward outcomes and coordinates discharge plan with client and family and other health professionals. What You’ll Bring: Master’s and/or Doctorate in Occupational Therapy from an accredited institution. One-year clinical Occupational Therapy experience preferred. Home Health experience also preferred. Current O.T. License in State of Maine & current CPR Card. Must have a reliable and insured automobile and a current Maine driver’s license.
