The Medical Social Worker manages case management for assigned patients, advocating for them and their families to navigate acute care episodes and monitoring resource utilization. They prepare patients for safe and timely discharge by utilizing family, community, state, and federal resources to support the patient's return to a productive life.
A Bachelor's degree is required, though a Master's degree is preferred, along with a Social Work license in West Virginia. Candidates should ideally have 5+ years of experience, with medical social work experience being preferred, and possess excellent communication and critical thinking skills.
