  • Type: Full TimeFamily First Health is seeking a Full-Time Licensed Mental Health Professional to join our Lebanon office as a Behavioral Health Consultant within our integrated primary care team. The Behavioral Health Consultant (BHC) works side-by-side with primary care providers and care team members to deliver brief, evidence-based behavioral health services in a fast-paced medical setting. Using the Primary Care Behavioral Health (PCBH) model, the BHC provides same-day consultations, brief interventions, and support for patients whose behavioral health needs impact their overall health and well-being. This role is ideal for a clinician who values team-based care, enjoys collaborating with medical providers, and is comfortable providing focused, solution-oriented care within the primary care visit structure.   • Overview: As a Licensed Mental Health Professional, you will work collaboratively as part of a multidisciplinary Primary Care team to address the mental and behavioral health needs of patients.
  • Your role will include: Provide same-day behavioral health consultations during primary care visits. Conduct brief functional assessments of behavioral health concerns. Deliver brief, evidence-based interventions (typically 1–6 visits).
  • Address common primary care behavioral health needs including: Depression Anxiety Stress Substance use Chronic disease self-management Sleep concerns Health behavior change (medication adherence, diet, exercise) Support patients with chronic medical conditions impacted by behavioral health factors. Provide crisis assessment and stabilization when needed. Work closely with Primary Care Providers (PCPs), nurses, care managers, and other team members. Participate in warm hand-offs and curbside consultations. Provide real-time recommendations to medical providers. Assist with behavioral health triage and care planning. Document consultations and interventions in the electronic health record (EHR). Maintain documentation that supports integrated, brief care delivery. Track patient outcomes and participate in population health management. Participate in team huddles and case consultation. Provide education to medical staff on behavioral health topics. Support development and maintenance of PCBH model fidelity.

Qualifications

Requirements
