Job Description

Physical Therapist, Home Health (Full-Time) Monday – Friday | 8: 00 AM – 5:00 PM

$10,000 Sign On Bonus

As a physical therapist, you will

  • Promote Elara Caring’s philosophy, mission, and administrative policies to ensure high-quality patient care
  • Provide direct patient care, evaluate functional needs and outcomes, and collaborate with interdisciplinary team members
  • Review patient progress and adjust care plans to ensure optimal outcomes
  • Develop and implement individualized plans of care based on comprehensive assessments
  • Deliver therapy services within established standards of care
  • Direct the assistance of support staff to help patients achieve functional goals
  • Maintain accurate documentation including evaluations, treatment goals, care plans, and physician orders
  • Provide leadership and serve as a resource to staff while promoting physical therapy services within the community

This position may require a physically demanding environment including lifting, transferring, and assisting patients, as well as the potential for occasional irregular hours if applicable.

What is required?

  • Graduate of an American Physical Therapy Association (APTA) approved physical therapy program
  • Current, unrestricted Physical Therapist license valid for the state of practice
  • Minimum one (1) year of experience in a healthcare setting
  • Ability and willingness to travel within the branch/office coverage area
  • Dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws
  • Ability to travel approximately 50%
  • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment

Reporting Structure

This position reports to the Branch Director, Clinical Manager, Clinical Supervisor, or designee.
