A Physical Therapist administers physician-prescribed physical therapy to patients intermittently in their residence to improve or minimize residual physical disabilities and return them to their optimum productive level. Responsibilities include initial assessment, plan of care development, modifications, and reassessments every 30 days, while participating in overall patient care planning.
Candidates must possess a degree in physical therapy from an accredited program and be currently licensed in the state of practice. Two years of experience in a home health setting and knowledge of Medicare OASIS documentation are preferred.

A Physical Therapist (PT) administers physical therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Services/Clinical Supervisor or Manager.

Provide physician prescribed physical therapy. Improve or minimize residual physical disabilities of the patient. Return the individual to optimum and productive level within the patient’s capability. Participate with all other home care personnel in patient care planning. Direct and supervise personnel as required. Responsible for initial assessment, plan of care, maintenance program development and modifications and reassessments every 30 days.
A passion to serve and help others live their best lives possible. Graduate with a Bachelor’s, Master’s, or Doctorate in physical therapy from a physical therapist curriculum approved by the Commission on Accreditation in Physical Therapy Education, American Physical Therapy Association, or the Committee on Allied Health Education and Accreditation of the American Medical Association, or the Council of Medical Education of the American Medical Association and American Physical Therapy Association. Currently licensed in the state in which practicing Two (2) years’ experience preferred in a home health setting Knowledgeable and experience with Medicare OASIS documentation Flexible work from home options available.
