This role involves partnering with patients, families, and the care team to coordinate care for patients with chronic diseases and manage effective care transitions across the continuum. The individual will also provide clinical health coaching to assist patients with self-management and promote utilization of community resources.
Candidates must have a High School diploma or GED, with post-secondary education and Medical Assistant certification preferred, alongside at least three years of medical experience, preferably in a chronic disease office setting. Desired experience includes knowledge of MCR Annual Wellness Visits, Transition Care Management, and Care Gap documentation, along with strong patient-centered communication and professional behavior.
Education: High School diploma or GED required; post-secondary education preferred, medical assistant certification preferred.
