Social Worker - RIACT

Liquid Personnel is recruiting an Adults' Social Worker to join its client’s Responsive Integrated Assessment Care Team (RIACT) based in Darlington.

What will your responsibilities be?

• In this role, you will,
• Undertake urgent assessments of adults in crisis to determine immediate care and support needs, aiming to prevent unnecessary hospital admissions.
• Work collaboratively with health professionals, reablement teams, and community services to implement short-term interventions that promote recovery and independence.
• Develop and review care plans within tight timeframes, ensuring safeguarding protocols are followed and outcomes are clearly documented.

Qualifications: & Experience:

• To be successful in this role you must have,
• Social Work England registration
• Eligible to work in the UK
• Hold a full UK driving licence
• Post-qualified experience
• Degree level or equivalent in Social Work
