Job Description

Liquid Personnel is recruiting Occupational Therapists for its clients Integrated Discharge Service; Occupational Therapy & Sensory Service Team located at Hackney.

Join our client's dynamic Hospital Team delivering high-impact occupational therapy interventions that promote independence and support safe, person-centred hospital discharges. This is a hybrid role with opportunities to work across London hospitals and in the community.

What will your responsibilities be?

  • Assess Hackney residents in hospitals across London for safe discharge planning
  • Collaborate with multi-disciplinary teams and attend ward meetings
  • Support residents with complex needs, including LD, MH, dementia, and NRPF
  • Conduct community reviews and Care Act assessments
  • Promote co-production and independence in daily living

Qualifications: & Experience:

  • To be successful in this role you must have,
  • HCPC-registered Occupational Therapist
  • Experience in hospital or social care settings
  • Strong assessment and Care Act review skills
  • Knowledge of safeguarding, MCA, and discharge planning
  • Excellent communication and problem-solving abilities
