Case Managers utilize therapeutic models to help clients understand and manage symptoms while conducting required program assessments and providing case management services in collaboration with treatment teams. Responsibilities also include connecting clients to prosocial activities, providing crisis intervention, teaching various life skills, and accurately documenting all services and contacts.
Candidates must possess a Bachelor's degree in human services or equivalent education plus work experience, along with at least two years of experience in a mental health setting preferred. A valid driver's license is required for this community-based role.

About the program: Eastern Region Service Center
The Eastern Region Service Center (ERSC) is a regional case management project that is administered by Advanced Behavioral Health to assist clients who are medically indigent or eligible for or currently receiving Medicaid LIA. CHR case managers work with the staff of 5 other agencies in the eastern portion of Connecticut to provide assessments and other case management services to clients with substance abuse and/or mental health issues. Case Managers engage, encourage and assist clients in obtaining treatment and housing as well as employment, education and medical services.
About the Position: Case Manager
Bachelor’s degree in human services preferred, or education plus equivalent work experience required.
At least two (2) years’ experience in a mental health setting preferred.
Valid driver’s license.

$18.8631/hour minimum, and up commensurate upon experience
*Actual rates are determined at the time of offer and are based on relevant experience and internal equity, as well as, adherence to our internal equity policy. Many of our positions offer program/location differentials and/or sign-on incentives. (Please note, for part-time positions this range would be pro-rated based on hours worked)
