The Case Manager will provide assessment and service planning to address Opioid Use Disorder and identified social determinants of health, developing and monitoring service plans while ensuring timely and accurate documentation in the electronic health record. This role involves serving as the primary point of contact for individuals, providing wraparound care, and communicating effectively with persons served and collaterals.
Candidates must possess a Bachelor's Degree in psychology or a related field, along with specialized training or experience in areas like child/adolescent development, substance use/dual recovery, and family treatment regarding psychiatric and substance abuse recovery. A strong understanding of population sensitivity, including HIPAA and privacy regulations, is mandatory.

We're in search of a case manager to work on a full time basis for our Recovery Now program, part of our Behavioral Health & Recovery Services Division. The case manager is accountable to the Program Director, who is responsible for providing supervision and training. The case manager is one who actively promotes the philosophy of the Gándara Center and maintains a high level of commitment to the program.
