About the role

Essential Responsibilities

  • Monitor and document patients' progress and response to treatment.

Case Management

Coordinate patient care activities and ensure continuity of care.

Facilitate communication between patients, families, and healthcare providers.

Advocate for patients' needs and assist in navigating healthcare systems.

Provide patient education on disease management, treatment options, and preventive care.

Clinical Coordination

Collaborate with physicians, specialists, and other healthcare team members.

Schedule and manage patient appointments, follow-ups, and referrals.

Ensure compliance with healthcare regulations and clinic policies.

Participate in quality improvement initiatives and clinical audits.

Administrative Duties

  • Maintain accurate and up-to-date medical records and case notes.
  • Prepare and submit reports on patient outcomes and case management activities.
  • Assist with the development and implementation of clinic protocols and procedures.

Job Qualifications • Education

  • Bachelor of Science in Nursing (BSN) or Associate Degree in Nursing (ADN) from an accredited institution.
  • Current and valid RN license, allowing practice in the State of Texas.
  • Experience:
  • Minimum of 2 years of clinical nursing experience, preferred.
  • Previous experience in case management or related field, preferred.
  • Skills:
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication and interpersonal abilities.
  • Proficient in electronic medical records (EMR) and other healthcare software.
  • Ability to manage multiple priorities and work in a fast-paced environment.
  • Compassionate and patient-centered approach to care.

Requirements

Candidates must possess a BSN or ADN and hold a current, valid RN license for practice in Texas, with a minimum of 2 years of clinical nursing experience preferred. Required skills include strong clinical assessment, critical thinking, excellent communication, and proficiency in Electronic Medical Records (EMR) software.
