The RN Care Manager will support patients in the care coordination program by identifying eligible individuals, reviewing charts for acuity levels, and overseeing comprehensive assessments and individualized care plan development. Key duties include supporting transitions of care, coordinating communication with community partners, and monitoring outcomes to recommend improvements.
Candidates must be a graduate of an accredited RN program with a current North Carolina RN License and hold a High School Diploma or GED. Required experience includes 2 or more years in primary healthcare, ambulatory care, and/or home health within the last five years, along with comfort navigating an EHR and a valid NC driver's license.
