As an LPN/LVN in Sub-Acute Care, the role involves providing professional nursing services under supervision, collaborating with the care team, taking vital signs, and contributing to resident care plans. The individual will also delegate tasks to staff and maintain a safe environment while treating everyone with dignity and respect.
Candidates must possess an active and unencumbered California LVN License and have successfully graduated from an approved LVN program, with knowledge of relevant nursing practices and regulations for long-term care. Previous experience in direct care or a Skilled Nursing environment, along with PointClickCare proficiency, is desirable but not mandatory.

As an LPN/LVN in Sub-Acute Care, you will work under the direct or indirect supervision of a Registered Nurse, providing professional nursing services in accordance with policies and procedures. You will collaborate with the care team, take vital signs, and contribute to the development of care plans for residents. You will also delegate tasks to facility staff and ensure a clean, safe, and welcoming environment for residents and their families. Above all, you will treat everyone with dignity, respect, compassion, and integrity.
