The Social Services Designee will assist residents and their families with adjusting to placement, illness, and the plan of care, aiming to help them achieve the highest practicable level of functioning per regulations. This role also involves supporting residents with treatment wishes, assisting with facility transitions, participating in council meetings, and supporting admissions and discharge planning.
A high school diploma or equivalent is required, along with the ability to work effectively with elderly individuals who may be physically, mentally, or emotionally ill. Candidates must also possess organizational skills, maintain confidentiality, and have knowledge of nursing practices and long-term care regulations.

Under the direction of the Social Services Director, our Social Services Designee will assist each resident and family in adjusting to placement, illness, and the plan of care, helping them attain the highest practicable level of functioning in accordance with current federal and state regulations, and our policies and procedures. You will assist residents and families with wishes regarding treatment and help with their transition and adjustment to the facility. You will also participate in resident and family council meetings as requested, and support Admissions and assist with discharge planning services.
