The Hospice Medical Social Worker will provide home-based social services, including counseling and resource planning, to patients managing difficult health issues. This role involves monitoring psychosocial conditions, conducting assessments, and collaborating with the hospice team on the patient's plan of care.
Candidates must possess a Master's degree in Social Work and hold an active MSW license in Kansas, along with current CPR certification. Essential qualifications also include knowledge of state and federal home health regulations and strong interpersonal skills.
