The Hospice Master's Social Worker will provide home-based social services, including counseling, community resource planning, crisis intervention, and advocacy, to patients facing challenges like depression or lack of resources. Responsibilities also involve monitoring psychosocial conditions, conducting assessments, documenting progress, and consulting with families and caregivers regarding the patient's plan of care.
Candidates must possess a Master's degree in Social Work along with an active MSW license in Kansas, and hold current CPR certification. Essential soft skills include strong interpersonal abilities, good communication, empathy, compassion, and resourcefulness.
