The Case Manager III serves as the primary support and case manager for individuals with mental illness and co-occurring disorders, assisting them to live as independently as possible within their chosen community. This role is distinguished from Case Manager II by requiring additional education, years of experience, and the ability to proficiently meet position standards.
Candidates must possess either a Bachelor's degree in Social Services or Human Services with one year of experience, or a High School/GED with four years of direct services experience in Mental Health or community services, with an Associate's degree substituting for two years of experience. A valid driver's license and personal vehicle insurance are mandatory for driving clients to appointments.

Full-Time | 10-Hour Shift | Shifts: Choice of 4 days/10hr, day off TBD based on program need | Days vary as needed
Expected starting wage range is $22.17 - $27.08. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.
