The Role

For this position, the Physician Assistant will manage high-risk patients in an outpatient clinic setting as well as providing in-home clinical support for patients in the community with the goal of preventing unnecessary acute care admission, providing holistic patient care, optimizing the management of the patients’ medical conditions, and providing support for clinical and psychosocial factors that contribute to the cyclical utilization of healthcare. What will you do as a Physician Assistant at GGH?
Assess patients' healthcare needs and provide clinical care and consultation
Perform an annual wellness exam for senior Medicare patients in a designated clinic or at their place of residence. Obtain and record the patient's medical history and symptoms, including medicine reconciliation
Perform HCC coding, verification, and risk adjustment
Coordinate continuity of care, prevention and avoidance of complications, and close clinical treatment and management under the direction of the patient’s primary healthcare provider(s) and collaboration with other referral sources

Counsel patients with education around their discharge plan
Execute the discharge plan and use your critical thinking skills to build an effective care plan during a critical transition period after a hospital discharge
Supports patients and their families, and provides compassionate care
Contribute to an interdisciplinary team (IDT) model and attend regular IDT meetings to collaborate with other healthcare professionals (social workers, case managers, and other clinicians) to ensure the holistic approach to the care plan is carried out
Collaborate with care team to ensure completion of holistic patient care
Ability to travel within the Medford, OR area to provide care to patients in their home based on the needs of business/market
Expected patient volume completed per day in clinic: 10-12
Expected patient volume completed per day in-home: 5-6
Complete charts within 72 hours of patients’ visits; you will have access to EMR and/or portals to review charts and document notes
Cover for other providers and Care Team Members when necessary
Maintains patients’ confidence by completing and safeguarding medical records, keeping patients’ information confidential, and abiding by all HIPAA rules and regulations
This position is a full time Physician Assistant opportunity (5 days a week, 7
30am-4:30pm PST schedule). This role can include in clinic, and in-home visits. Home visits will be scheduled within a 30 mile radius of your home region (Medford, OR).

Current Oregon PA license
2 + years of clinical experience as a PA in either Primary Care, Family Medicine, Internal Medicine, Gerontology or Palliative Care.
Acute care experience with high-risk patients is a plus
Home health experience is a plus
Experience with senior populations is a plus
Experience in Medicare Risk Adjustment
Experience working in a team-based environment
Physician Assistant certification from NCCPA
Working knowledge of chronic care management and various medication options
Current DEA licensure / Prescriptive Authority required; if need to obtain, GGH will reimburse after 90 days of employment ONLY if purchased after date of hire
Active CPR/BLS Certification
Current Oregon Driver’s License
Willing to sign collaborative agreement with our Medical Director
