The MDS Nurse plans and organizes the MDS/RAI process, ensuring compliance with all State and Federal MDS transmission requirements in coordination with the RAI/PPS Coordinator. This role involves collaborating with clinical and administrative staff regarding the Resident Assessment Instrument (RAI) process, completing assigned assessments accurately, and tracking submission status.
Candidates must possess an active LPN or RN license and have a minimum of two years of nursing experience in a skilled nursing facility, with at least one year working directly with MDS strongly preferred. Required certification is RAC-CT, which must be obtained within six months of hire, alongside the ability to obtain licensure in Colorado, Florida, and Minnesota.
We are seeking an MDS Nurse to join our clinical team at Anoka Homestead Rehab & Living Center. The MDS Nurse is responsible for planning and organizing the MDS/RAI process in coordination with the RAI/PPS Coordinator. This position assures compliance with all State and Federal MDS transmission requirements. The MDS Nurse is responsible for collaborating with Clinical and Administrative staff regarding issues relative to the Resident Assessment Instrument (RAI) process.
$31.25-$45.67 (Based on years of experience)
Active and unencumbered license as a Licensed Practical Nurse (LPN) or Registered Nurse (RN). Ability and willingness to obtain licensure in Colorado, Florida, and Minnesota.
Minimum of two (2) years of nursing experience in a skilled nursing facility (SNF). At least one (1) year of experience working directly with MDS is strongly preferred.
Resident Assessment Coordinator – Certified (RAC-CT) certification required or must be obtained within six (6) months of hire.
