The Physical Therapist will provide services to home health clients with disabilities to maximize functional independence by evaluating clients and implementing individualized plans of care. Responsibilities also include documenting care, coordinating with the team, and overseeing Physical Therapist Assistant activities.
Candidates must possess a Bachelor's Degree in Physical Therapy and a valid Physical Therapist license in Nebraska, along with CPR certification and a valid Nebraska Driver’s license. One year of experience as a Physical Therapist and geriatric experience are preferred.

The Physical Therapist is responsible for providing Physical Therapy services to Tabitha’s Home Health Care Seniors/residents/clients with physical or cognitive disabilities to maximize functional independence.
