The Wraparound Facilitator will work with families of children with serious emotional disturbances by facilitating a planning process that is family-centered and strength-based. Responsibilities include delivering intensive case management, accurately documenting services, facilitating team meetings, and communicating participant progress to relevant parties.
Candidates must possess a Bachelor's Degree in social work, human services, or a related field, and be proficient in Microsoft 365, with EHR experience being a plus. Applicants are required to have reliable transportation, a valid driver's license, and current auto insurance coverage.
