The Community Health Worker will assist patients in navigating the health system at various levels and communicate identified barriers to superiors. They will also provide basic education on screenings, chronic disease self-management, and wellness.
Candidates must possess a High School diploma or GED, advanced computer and data entry skills, and a valid driver's license. Successful candidates must also complete required professional skills training, health coach training, and obtain necessary certifications within specified timeframes.
Community health worker: 1 – Full-time opening ( 4 day work week with one 12 hour day)
