The RN Health Coach will educate patients on chronic disease management and prevention, assisting them in setting and achieving self-management goals. Responsibilities also include contacting insurance companies for preauthorizations and instructing diabetic patients on meter use and insulin.
Candidates must possess a minimum RN license, with a BSN preferred, along with current BLS and Mandatory Reporter certifications. Preferred qualifications include clinical office and case management experience, and obtaining Certified Diabetes Educator certification within six months.
Rn health coach: 1 – Full-time opening ( 4 day work week with one 12 hour day)
