The After-Hours Palliative/Comfort Care LVN provides skilled, compassionate, end-of-life care and symptom management to patients in their homes or facilities during evenings, nights, and weekends. Key duties involve responding to urgent calls, conducting nursing assessments, updating care plans, and offering emotional support to families.
Candidates must possess an active California LVN license, with a preference for at least one year of general nursing experience and six months or more in hospice and/or palliative care. Successful applicants must also clear DOJ/FBI fingerprinting and a drug screening.
Takes on-call duty nights, weekends and holidays as assigned. Provide specialized care to patients who are in the final stages of their life. Work closely with patients, their families, and a multidisciplinary healthcare team to ensure comfort and support during this challenging time. Provides nursing services, treatments and diagnostic and preventive procedures as assigned. Observes signs and symptoms and reports to the physician and RN reactions to treatments, including drugs and changes in the patient’s physical or emotional condition. Teaches and counsels the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home. Evaluates with registered nurse the effectiveness of the LPN’s/LVN’s nursing service to the patient and family under the guidance of the registered nurse. Maintains accurate and complete records of observations, treatments and care of patient. Participates in medical record audit as assigned. Attend staff meetings, IDG conferences and in services as scheduled Is responsible for submitting any changes in schedule to Director or Manager of Patient Services on a daily basis Prepares clinical and progress notes Assists the physician and RN in performing specialized procedures Qualifications and Requirements: Active California LVN license Minimum of one (1) year in nursing preferred Six months or more experience in hospice and/or palliative care preferred Skills and Knowledge: Acute Patient Assessment: Mastery in head-to-toe physical exams to identify subtle changes in condition, especially when patients are non-verbal. Pain & Symptom Management: Expertise in administering comfort medications (pharmacological) and non-pharmacological interventions to alleviate breakthrough pain, nausea, and restlessness. Technical Nursing Procedures: Proficiency in managing specialized equipment such as subcutaneous medication pumps (CAD pumps), Plurex drains, and Foley catheters. Wound & Skin Care: Skilled in assessing pressure ulcers and applying appropriate dressings to maintain dignity and comfort. Medication Reconciliation: Regularly updating and verifying medication lists to ensure alignment with the hospice care plan and avoid adverse interactions. EHR Proficiency: Accurate and timely documentation in Electronic Health Record systems to ensure continuity of care with daytime teams. #ZR St. Paul’s Benefits: Health, dental, vision, life Same day pay available!
