This role involves advocating for individuals' rights and needs, serving as a primary contact between families/advocates and residential staff, and maintaining comprehensive records for the assigned caseload. The professional will also participate in the Interdisciplinary Treatment Team to plan, implement, and monitor treatment plans, including staff training.
Candidates must possess a Bachelor's Degree in a human services-related field and have a minimum of two years of experience working with individuals with intellectual and developmental disabilities, including knowledge of the NYS OPWDD Service system. Essential qualifications include proficiency in Microsoft Office and EHR systems, excellent communication skills, and the ability to maintain flexible hours.
