The role involves facilitating patient encounters with the hospital, focusing on insurance processes, discharge planning, and ensuring appropriate resource utilization. This position also includes advocating for palliative care services when necessary and providing education to healthcare professionals, patients, and families.
Candidates must possess three years of patient care experience that encompasses patient assessment, treatment details, and lab value interpretation, maintaining current clinical knowledge. Experience in palliative care is preferred but not mandatory.
Facilitate the patient encounter with the hospital, specifically in reference to the insurance process, discharge planning, and appropriate utilization of resources. May service as palliative care services advocate for patients as appropriate. Provides education for healthcare professionals, patients and their families. Participates as a member of the case management team.
Three (3) years patient care experience encompassing patient assessment, treatment, lab values, etc with current clinical knowledge. Palliative care experience may be preferred.
