The Physical Therapist will be responsible for delivering physical therapy care to adult and geriatric patients within a diverse clinical team setting. Key duties include preparing necessary documentation such as progress notes and discharge summaries, and establishing individualized plans of care.
Candidates must possess a valid State Physical Therapist License and relevant credentials to perform the role. Experience in Skilled Nursing Facilities (SNF) and/or Acute Care settings is required for this position.
We are hiring a PRN Physical Therapist to deliver care to our patients and work with a diverse team of clinical professionals.
Provide physical therapy to adult and geriatric patients Prepare documentation, progress notes, and discharge summaries Establish a plan of care in accordance with patients' needs and capabilities Travel to nearby centers as needed
