The RN Care Coordinator centrally manages patient care by strategically assessing, planning, and facilitating comprehensive care across the continuum to ensure optimal outcomes and smooth transitions. This involves collaborating with multidisciplinary teams, physicians, and post-acute providers while advocating for patients and mitigating readmission risks.
Candidates must be graduates of an accredited school of nursing and hold an RN license. A minimum of two years of acute hospital clinical experience is required, or a Master's degree in Case Management or Nursing can substitute for one year of experience.
