The RN Coordinator is primarily responsible for assessing and coordinating home health, hospice, and infusion services for hospitalized or outpatient patients, focusing heavily on coordinating Infusion referrals and educating patients/caregivers on infusion devices. This role involves functioning as an integral part of an interdisciplinary team, providing community education, and interfacing with various external organizations and payors.
Candidates must possess a Bachelor of Science in Nursing (BSN) and hold current RN licensure in North Carolina or a compact state, along with Basic Life Support (BLS) certification. A minimum of three years of nursing experience is required for this position.

Pursue your passion for caring with Duke HomeCare & Hospice, which offers hospice, home health, and infusion services across the region, as well as serves as the home forthe Duke Caregiver Support Program. Team members work closely with a patient's physician to provide comprehensive, individualized care in the comfort of their home or at our inpatient hospice facility in Durham, NC.
Job Summary The Coordinator is a dynamic, fast paced position responsible for coordination of home health, hospice, and infusion services for adult and pediatric patients across the inpatient setting in the 3 Duke Hospitals. The primary focus is assessment of home needs and education of patients and caregivers related to the safe and effective use of home infusion devices, hospice and HH services. The coordinator works closely with the interdisciplinary teams collaborating with referral sources and community partners to ensure seamless, high‑quality care transitions.
The ideal candidate has the ability to navigate complex care environments, build strong relationships across disciplines, and support seamless coordination between hospital systems and community‑based services.
Float to all 3 Duke hospitals in the Research Triangle area.
M-F (5) 8 hour days-Rotating call schedule every 4-6 weeks to cover weekend staff on PTO. Some Holiday Coverage
Education, Degrees and Formal Training
BSN required. Current RN licensure in the state of North Carolina, or compact licensure in participating state is required.
BLS required, but if not current may be renewed during orientation.
Three years of nursing experience are required.
