The Clinical Admissions Nurse is responsible for representing hospice programs to interested parties through assessment, information sharing, and admissions in various healthcare settings. This role involves communicating with referral sources and physicians to ensure referral appropriateness and obtaining necessary certifications.
Candidates must be graduates of an accredited nursing program and hold the required state of Ohio Nurse Licensure, along with a valid, insured driver's license. A minimum of one year of nursing experience is required, preferably in intake, home care, hospice, and/or oncology nursing.
00a-9:30p. You will be scheduled for 24 hours and paid for 30 and be eligible for full-Time benefits

The Clinical Admissions Nurse is responsible for assuring for assuring that HWR programs are effectively and responsibly represented to patients, families, and others who have interest in hospice services through assessment, information sharing and/or admissions in a patient’s home, nursing facility, or in a health care facility. This position works closely with the Referral Department and other referral sources to promote the hospice philosophy of care.
