Care Navigator

This role supports residents living with dementia and their caregivers through the CMS GUIDE Model by providing person-centered care via comprehensive assessments and individualized care planning. The Care Navigator acts as a key liaison among medical providers, behavioral health professionals, wellness teams, and caregivers to ensure seamless support and improved outcomes.

Candidates must have at least five years of experience in care coordination or case management specifically working with individuals living with dementia and their caregivers. A bachelor’s degree in social work, nursing, or a related field is required, with a master’s degree being preferred.
