About the role

This role supports residents living with dementia and their caregivers through the CMS GUIDE Model by providing person-centered care via comprehensive assessments and individualized care planning. The Care Navigator acts as a key liaison among medical providers, behavioral health professionals, wellness teams, and caregivers to ensure seamless support and improved outcomes.

What We’re Looking For… Shell Point Retirement Community is seeking a compassionate and experienced Care Navigator to support residents living with dementia and their caregivers through the CMS GUIDE (Guiding an Improved Dementia Experience) Model.

Based in the primary care clinic, this role partners with an interdisciplinary healthcare team to provide person-centered dementia care through comprehensive assessments, individualized care planning, education, and coordination of services.

The Care Navigator serves as a key liaison among medical providers, behavioral health professionals, wellness teams, and caregivers to ensure seamless support and improved outcomes.

Ideal candidates will have a bachelor’s degree in social work, nursing, or a related field (master’s preferred), at least five years of experience in care coordination or case management working with individuals living with dementia and their caregivers, and strong communication, assessment, and problem-solving skills.

This role is dedicated to enhancing quality of life for individuals living with dementia, reducing caregiver strain, and helping residents remain safely in their homes.

Requirements
