We are growing and seeking an experienced, compassionate Home Health Registered Nurse (RN) to join our team on a per diem basis in the Inland Empire area!

Key Responsibilities
Develop and implement individualized plans of care in partnership with patients, caregivers, and interdisciplinary teams.
Provide preventative and rehabilitative nursing procedures as ordered by the physician.
Deliver patient, caregiver, and family education and counseling.
Prepare and maintain accurate clinical notes.
Communicate effectively with physicians and healthcare team members regarding patient needs and changes in condition.
Identify discharge planning needs and implement prior to patient discharge.
Supervise Licensed Practical/Vocational Nurses (LPN/LVN) and Home Health Aides (HHA/CNA) as directed.
Participate in on-call duties, quality improvement programs, and in-service training.

Candidates must possess an active California RN License and a minimum of two years of nursing experience, with home health experience being preferred. A reliable vehicle, valid driver's license, current CPR certification, and strong clinical and communication skills are also mandatory.
