Key Responsibilities Develop and implement individualized plans of care in partnership with patients, caregivers, and interdisciplinary teams.
Provide preventative and rehabilitative nursing procedures as ordered by the physician.
Deliver patient, caregiver, and family education and counseling.
Prepare and maintain accurate clinical notes.
Communicate effectively with physicians and healthcare team members regarding patient needs and changes in condition.
Identify discharge planning needs and implement prior to patient discharge.
Supervise Licensed Practical/Vocational Nurses (LPN/LVN) and Home Health Aides (HHA/CNA) as directed.
Participate in on-call duties, quality improvement programs, and in-service training.
