• Area: Coachella Valley What You’ll Do The Physical Therapist (PT) is responsible for delivering high‑quality physical therapy services in accordance with the physician’s plan of care and all requirements outlined in the Personnel Services Agreement. The PT plays a key role in patient recovery, functional improvement, and interdisciplinary care coordination. Provide skilled physical therapy services based on the written physician’s plan of care Conduct comprehensive, ongoing assessments and collaborate with the patient, caregiver, and interdisciplinary team to update and individualize the plan of care Educate and counsel patients, caregivers, and families on therapy goals, safety, and home exercises Prepare accurate and timely clinical documentation, including visit notes and progress summaries Communicate regularly with the physician and healthcare team regarding patient progress and changes in condition Initiate and implement the physical therapy program, instructing caregivers and appropriate staff on techniques they can safely perform Participate in case conferences to review goals, progress, and care plans Train patients in the proper use of prosthetics and assistive devices Identify needs for additional home health services and coordinate referrals when appropriate Participate in patient discharge planning and recommendations for continued care Provide in‑service education and training to agency staff as needed Engage in peer consultation and quality improvement processes Supervise Physical Therapist Assistants (PTAs) in accordance with state and organizational guidelines Requirements Active CA Physical Therapy License Strong verbal and written communication skills Strong organization skills Possesses and maintains current CPR certification Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order Why Join Bella Terra Home Health & Hospice of the Desert?
