The primary role involves functioning as part of an interdisciplinary team, focusing on initial discharge planning and coordinating care for members in inpatient facilities. This includes assisting with member discharge evaluations and collaborating on treatment plans aimed at promoting long-term health and preventing relapse.
require a Master's or Bachelor's degree in a behavioral health field, or a Bachelor's/Associate's degree in a non-related field with relevant experience, or a high school diploma with three years of related experience. Candidates must also maintain current CPR, First Aid, and Therapeutic Options certifications, possess a valid Arizona driver's license, and be at least 21 years old.

Driving and Other Requirements
Must have and maintain a valid Arizona driver’s license, have a registered and reliable vehicle that meets Arizona state law standards, and meets the insurance standards of Southwest Network. Position requires travel within the community.
