Providers act as hands-on caregivers transforming patient health through a relationship-centered, high-touch approach focused on value-based care. Responsibilities include delivering exceptional care in an established outpatient practice setting, seeing approximately 18 patients per day.

Praxis Health is looking for a family physician who wants to practice the way medicine should feel: a manageable panel, no hospital rounding, specialist support at your side, and a team that handles the administrative weight so you can focus on your patients.   • Year two production average: $320,000

Practice details

  • You will step into a fully established outpatient panel, with no building from scratch. An endocrinologist is embedded within the clinic, giving your patients specialist access without the typical referral friction.
  • Panel: 100% established, outpatient only, No OB
  • Schedule: 36 patient contact hours over 4-day workweek, 18 patients per day
  • Specialist access: Endocrinologist on-site
  • Support model: Physician autonomy backed by robust administrative support
  • Call: Shared only, no hospital call requirements

Minimum Qualifications

  • MD or DO
  • Board certified or board eligible in Family Medicine

  • The following physical requirements are included per standard credentialing and HR documentation.
  • While performing the duties of this role, the employee is regularly required to stand, walk, use hands to handle or feel, reach with hands and arms, and talk or hear.
  • The employee is occasionally required to sit, stoop, kneel, or crouch.
  • The employee must regularly lift or move up to 10 lbs, frequently up to 25 lbs, and occasionally up to 50 lbs.
  • Work environment is a combination of medical office and exam/procedure room settings.

Requirements

Candidates must possess an MD or DO degree and be Board Certified or Eligible in Family Medicine. The role requires regular physical activity including standing, walking, reaching, and lifting up to 50 pounds occasionally.
