The Hospice Nurse provides skilled nursing assessment, planning, and end-of-life care under the direction of the Medical Director and attending LIP, focusing on maximizing patient comfort and supporting families. Responsibilities include identifying patient/family needs, developing individualized plans of care, performing comprehensive symptom assessments, and providing direct supervision of Home Health Aides and/or LPNs.
Candidates must be a Registered Professional Nurse currently licensed in New York with knowledge of nursing theory equivalent to an associate degree, plus one year of nursing experience in an acute care setting. A valid driver's license and reliable vehicle are required, while a Baccalaureate Degree and/or Hospice and Palliative Care Nursing Certification are preferred.

Under the direction of the interdisciplinary team, the Hospice Nurse works under the general direction of the Hospice Medical Director and the attending Licensed Independent Provider (LIP) in the provision of skilled nursing assessment, planning and care to maximize the patient’s comfort at end of life and support the patient’s families/ primary caregivers. The Hospice RN is responsible for identifying patient/family needs and for providing supportive care in accordance with the attending LIP’s orders, the interdisciplinary plan of care and Guthrie Hospice policies and procedures. Responsible for providing direct supervision of Home Health Aides and or LPN’s providing care for Guthrie Hospice. Education, License & Cert: Registered Professional Nurse currently licensed in New York. Knowledge of nursing theory and practice at a level normally associated with an associate degree. Baccalaureate Degree and/or Hospice and Palliative Care Nursing Certification preferred. Education and experience requirements may vary according to the combined background of the individual. Requires valid driver’s license and reliable vehicle.
