Conduct comprehensive adult and geriatric assessments for hospice patients to evaluate and manage their plans of care. Maintain accurate documentation and ensure continuity of care within a collaborative hospice environment.
Must hold current, unrestricted RN and NP licensure in the state of practice. Board certification by AANP or ANCC in adult, family practice, geriatric, or adult-gerontology primary care is required.
