The Outreach Worker provides formal and informal outreach services to clients, including assessments and the development of individualized care plans. They also offer counseling and case coordination services to ensure clients receive the necessary support.
Candidates must have a high school diploma and at least one year of experience, preferably with the homeless or mental health population. Strong assessment, case management, and communication skills are required, along with the ability to drive and maintain office cleanliness.
OUTREACH WORKER provides formal and informal outreach and other clinical services to O'Hare Outreach clients.
