OUTREACH WORKER provides formal and informal outreach and other clinical services to O'Hare Outreach clients.
Provide coordinated formal outreach services to prepare potential clients to accept services and formal outreach services to triage and register clients.
Provide initial medical/psychological and additional assessments to registered clients leading to the definition of treatment issues, the development of an individualized care plan and the implementation of a firm referral.
Provide counseling and case coordination services to follow through on referrals and monitor client responses to services. May function as a case manager for clients when necessary.
Maintain and protect the health and safety of clients and O'Hare occupants within the limits of the position holder's capabilities. Maintain office cleanliness.
Provide documentation in logs and client records of services delivered. Assist in the coordination of records, building databases on current program files.
Other duties as required for effective teamwork or as assigned by O'Hare Outreach Supervisor.
