Nephrology Nurse Practitioner - Medical Office:

2 days a week @ Apple Valley Care Center (Apple Valley, CA 92308 ) + 3 days a week @ Upland Care Center (Upland, CA 91786)
The Nurse Practitioner Specialty is responsible for providing for the health care needs of patients in collaboration with a physician, through advanced assessment and identification of health states including abnormal conditions, diagnosing, treatment therapies and/or evaluation. Performs care in the specialty area of nephrology and patients on dialysis.
Utilizes evidence-based clinical practice standards to provide direct clinical services for patients within prescribed guidelines and physician instructions.
Acts independently to perform physical assessments, diagnostic functions, specialized risk assessments, and make critical decisions regarding a wide variety of health situations, including high risk and crisis situations.
Acquires patient health histories and evaluates patient, performing physical examinations.
Orders, interprets and evaluates diagnostic tests to identify and assess patients' clinical problems and health care needs.
Discusses case with physician.
Formulates and documents care plan.
Prescribes medication, therapies and/or other forms of treatment.
Evaluates the patient’s response to therapy.
