The Hospice RN Case Manager provides professional nursing care to hospice clients as prescribed by the physician, taking full responsibility for client assessment, care planning, therapeutic intervention, and overall supervision of client care and outcomes. This role involves managing patient symptoms in collaboration with the physician and interdisciplinary team, assisting with family/caregiver grief issues, and providing functional support to Aides and LPN/LVNs.
Applicants must be a graduate of an approved nursing education program and hold a Registered Nurse license in the State of Pennsylvania. A minimum of 2 years of experience as a professional nurse in acute care, home care, or ER is required, with home care experience being preferred but not mandatory.
