The Registered Nurse will provide initial assessments of newly referred patients in the home care setting and collaborate with patients to create patient-centered plans of care. Responsibilities also include continuous reassessment of patient needs and supervision and training of Personal Care Assistant/Home Health Aide staff.

The pay range displayed on this job posting reflects the anticipated range for this position.
