Commonwealth Home Health Network is looking for Registered Nurses to join our team. CHHN provides skilled and unskilled services to seniors, older adults, and those who are disabled. Provide skilled care to our patients allowing them to return to independence. Conduct and complete initial assessment of our patients in order to generate a personalized plan of care in collaboration with physicians and the interdisciplinary team. Educate our patients and their families on the next steps in continuing care and improvement of day-to-day life. Coordinate and perform skilled treatments to enhance the well-being of the patient with the goal of enhancing patient health. Qualifications Valid registered nursing license (required) 1+ years of registered nursing experience Associate degree in nursing (required) Home health experience is a plus.
