Commonwealth Home Health Network is looking for Occupational Therapists to join our team. CHHN provides in-home, skilled and unskilled services to seniors, older adults, and those who are disabled. Provide care for our community of recovering patients, ushering them back to a life of independence. Create and follow personalized plan of care in collaboration with RN and the interdisciplinary team. Educate our patients and their families on next steps in continuing care and improvement of day to day life including: therapy, exercise, and treatments. Coordinate and perform medical treatments to enhance the well-being of the patient with the goal of enhancing patient health. Focus on the goal of improving the overall health of our community. Oversee Occupational Therapy Assistants. Perform all duties with a passion for compassion. Qualifications Valid Occupational Therapy license (required) 1+ years of OT experience A heart for providing the best care to our community
