The Targeted Case Manager will assist individuals in gaining access to recovery supports and services. Responsibilities include assessing needs, coordinating services, and documenting outcomes.
A Bachelor’s degree in social work or a related field is required, along with 1 year of post-graduate work experience or a Master's Degree. Candidates must also possess a current driver’s license and basic computer skills.
