Work Schedule: Full-time, In-person in various senior settings with some opportunities to work from home part of the time.
Active compact Registered Nurse (RN) license in Washington or Idaho. Must possess a valid and current driver's license. Strong background in geriatric/senior care, with a focus on care assessments and care coordination. Experience in care planning for chronic conditions, including collaboration with interdisciplinary teams. Excellent documentation and electronic medical record-keeping skills. Ability to work autonomously, manage ambiguity, solve problems, and adapt to new processes as the role and organization evolve. Experience in working in senior living facilities is highly desirable. Skills & Competencies: Strong clinical assessment and care planning skills. Ability to work closely with both clinical and facility teams to coordinate care. Proficiency in using EHR systems and other technologies for documentation and reporting. Must possess a valid and current driver's license & be comfortable driving around to different senior home settings including senior living communities as well as individual homes and beyond. Excellent communication skills and the ability to work collaboratively with a diverse team. Adaptability in a new role and organization, with a focus on innovation and resident care.
