The Nurse Navigator provides individualized assistance, support, and comprehensive care coordination for patients and their families to eliminate barriers to timely care and facilitate system flow. This role involves serving as the first point of contact, maintaining provider interaction, and collaborating with the multidisciplinary team to deliver person-centered care for IDD patients.
Candidates must possess a Bachelor of Science in Nursing (BSN) and have a minimum of three years of acute care experience. The role focuses on working directly with unique Intellectual and Developmental Disabilities (IDD) patients and their families within the Complex Care Center.
GENERAL PURPOSE: Provides individualized assistance, support and comprehensive coordination of care for patients and their families/caregivers, to eliminate barriers to timely care, facilitate flow through system, facilitate interactions with care team, increase patient and provider satisfaction, maintain point of contact with providers and serve as first point of contact for patients and families. Full Time Days, no weekends, no holidays Join our Growing team the Complex Care Center, the newly, state-of-the-art Golisano Intellectual and Developmental Disabilities Institute (IDD) this role will be working directly with our unique IDD patients and families. The Nurse Navigator focuses on utilizing available resources, improving access to care, best practices, advocate, disease management, and to collaborate with the multidisciplinary team to deliver the very best person-centered care for our IDD unique population. BSN in nursing required, 3 years of acute care experience required.
