The LPN will ensure that patients receive high-quality healthcare by obtaining vitals and medical histories, managing the nursing phone line, and performing blood draws. They will also handle administrative tasks such as ordering medical supplies and maintaining a clean workspace.
Candidates must have a valid New York State LPN license and experience working with underserved populations. Strong communication skills and a commitment to culturally competent care are essential.
Expected schedule will start at 8:30 am some days and extend into evenings on others Delivering quality healthcare in a Harm Reduction setting Checking Prescription Monitoring System for controlled substances daily for Providers Obtaining Vitals and accurate medical history on all patients prior to them being seen by their provider Maintaining a clean and functional workspace Checking inventory and stocking exam rooms with medical supplies Answering the nursing phone line with a maximum of 24 hour turnaround time to patients who have called in for either primary care or medication needs Entering accurate medical information into REACH Medical's electronic medical record system Completing all required blood draws Practice within NYS Healthcare regulations and REACH Medical policies and procedures Hepatitis C treatment and follow up procedures PrEP and PEP protocols Understand and be cross trained in many non-clinical work procedures within the practice (ie answering phones, completing prior authorizations) Reporting to Associate Medical Director, and Practice Manager for Nursing administrative tasks and assignments. Qualifications Experience working with people with lived experience of health inequity. Valid New York State LPN license Willingness/ability to learn EMR system and workflow processes Ability to work weekends and evenings as required Must possess strong intrapersonal skills with a commitment to provide respectful, culturally-competent communication to fit the context of our target population. Clear and detailed understanding of HIPAA policies (confidentiality) to ensure compliance on all public-facing publications including on social media, live events, and print materials Stellar communication skills and ability to work well within multidisciplinary agency Demonstration of both significant experience in and commitment to providing care to underserved populations as the foundation of REACH Medical is the provision of harm reduction-based, trauma-informed, low threshold care without stigma.
