The Nurse Practitioner or Physician Assistant will develop patient-centered care plans, assess patient health, and coordinate care for individuals experiencing homelessness. They will also document care accurately and collaborate with a multidisciplinary team to improve health outcomes.
Candidates must be graduates of an accredited program for Nurse Practitioners or Physician Assistants and maintain licensure in Arizona. A minimum of three years of experience in Family Medicine, Internal Medicine, or Emergency Medicine is required, along with skills in working with underserved populations.
